Minor Release Forms
Tattoos
If you are 16 and up you can legally be tattooed with parent/guardian consent, however not all artists at Classic Ink will tattoo minors under the age of 18. By law, the Florida Department of Health requires a notarized consent form to be completed. Parent/guardian AND minor must have a valid, government-issued ID at the time of every service. Parent/guardian on the form must be present and accompanying the minor during the entire duration of the tattoo. We cannot and will not tattoo a minor on or around any area that would typically be covered by a swimsuit.
